I genuinely think it might be tied to the fact the characters are so old. I don't know for certain if that would have a definitive impact on your ability to use in game mechanics, but again, seven years is a /long/ time. You might be able to file an asset transfer on the forums (here: https://forum.gta.world/en/forum/564-asset-transfer-request/) and explain your situation, sure.

You can try to go into your Rage settings and disabling CEF rendering. I do believe that can conflict with the surgery script as I've heard that several times. As for the faces being different, I've not heard that.. but my guess would be six (almost seven) years of updates likely conflicted with something and potentially reset your sliders. Let me know if the first suggestion works for you.
